2 year follow-up
We would like to keep in contact with all the families involved. This will enable us to complete the follow-up phase of the study and assess your child's health and development at about two years of age.
So that we can get a complete picture of the study findings, we need to gather as much information on as many of the children as possible, even if you have no concerns about your child's development at that time. In order to do this we will send a questionnaire for you to complete about your child's health. The 2 year follow-up questionnaire can be viewed and downloaded below.
To help keep a note of the information we will be interested in later, please use the Baby-OSCAR record card. You can keep this with your baby's 'Personal health record' – the red book, which the hospital gave to you soon after your child's birth.
